This meeting is to verify your CHROME patient records before the patient leaves the office. This will stop the need of having patients return at a later date to retake certain records. Once verified these records can be submitted along with your CHROME Rx.

All staff members listed on the form can verify the records.

Join the meeting via:

• COMPUTER / LAPTOP – ensure all your records are on your computer desktop and able to be shown easily
• CELLPHONE – ensure all records are easily accessible and able to be shown via your cellphone camera
