Step 1 of 3: Generate CombiGuide RX

Steps:

• Complete this form and click “Submit RX and go to Records Upload”. The RX will be emailed to you and uploaded to ROE.
• A second page will appear that allows you upload your records. Once uploaded, the case is submitted in full.
• The final step will be to schedule a meeting with one of our specialists to get your case started.
