Welcome to the new CHROME GuidedSMILE™ RX.

Steps:

1. Complete this form and click “Submit RX and go to Records Upload”. The RX will be emailed to you and uploaded to ROE.
2. A second page will appear that allows you upload your records. Once uploaded, the case is submitted in full.
3. The final step will be to schedule a meeting with one of our specialist to get your case started. 

Follow the status of submitted cases via your online account. Status not immediate.
