Steps:

Complete this form and click “Submit RX and go to Records Upload”. The RX will be emailed to you and uploaded to ROE.
A second page will appear that allows you upload your records. Once uploaded, the case is submitted in full.

Follow the status of submitted cases via your online account. Status not immediate.
