Submit CombiGuide Case

Steps:

1. Complete this form and click “Submit Rx and go to Records Upload”. The Rx is emailed to you and uploaded to GuidedSMILE.
2. A second page will appear that allows you to upload your records. Once uploaded, the case is submitted in full.
