What are the warranty, lab turnaround, case submission, and ADA codes for a PFZ restoration?

This article covers the administrative details for ordering and billing a PFZ (Porcelain Fused Zirconia) restoration with ROE.

Warranty

The PFZ carries a 2-year warranty. For full terms and conditions, see ROE's warranty statement.

Lab turnaround

PFZ cases require 9 days in the lab, which does not include shipping time. To schedule the patient's return appointment accurately, use ROE's case delivery calculator, which accounts for the in-lab window and transit.

Submitting your case

You can submit digitally or ship physical records.

Digital submission Submit scans through your intraoral scanner's portal (search for ROE Dental Lab), or export your STL files and upload them. You can upload files to ROE or create or log in to your ROE account to start a new case, or send a case directly.

Analog submission Print and complete the appropriate Rx from ROE's instructions, Rx's, and forms page and include it with your case. For prepaid shipping, use ROE's UPS label and case pick-up service.

ADA insurance codes

Select the code that matches the restoration type. ROE lists the following codes for PFZ. Confirm coverage with the patient's plan, since payer rules vary.

ADA code Description
D2740 Crown, porcelain/ceramic substrate
D6058 Abutment-supported porcelain/ceramic crown
D6065 Implant-supported porcelain/ceramic crown
D6740 Crown, porcelain/ceramic (bridge)
D6245 Pontic, porcelain/ceramic (bridge)
D6068 Abutment-supported porcelain/ceramic (bridge)
D6075 Implant-supported porcelain/ceramic (bridge)

ROE maintains a fuller ADA codes reference for dental procedures.
