What records and Rx details does ROE need to start a Bespoke case?

A Bespoke case begins with a Bespoke Rx plus your patient records. The Rx is available in digital and paper form. Incomplete records are the most common cause of avoidable delay, so use the breakdown below as a pre-submission checklist.

Required Rx fields

Section Required fields Optional fields
Doctor and patient Doctor name, email, phone, address, city, state, ZIP, patient name, patient age Date, sex, Call Me request
Scheduling Return by 5:00 p.m. date Expedite (fees apply), shipping supplies request
Implant Implant brand, implant line or platform None
Tooth Base shade (VITA) Tooth numbers to omit, bite class
Authorization Signature, license number None
Instructions Doctor and patient goals None

Do not skip the instructions field. "Please indicate Dr. and patient goals" is a required field for a reason. This narrative is what the design and finishing teams work from when a selection alone does not resolve a judgment call.

How to submit

Step 1: Open the Rx. Visit www.roedentallab.com/bespoke and select Submit Rx, or go directly to the Bespoke Rx form.

Step 2: Complete the online form. Drag and drop any relevant case photographs into the upload field under Final Notes. Photographs are among the most valuable records you can send on a Bespoke case, because they let the team interpret the patient's face rather than just the arch.

Step 3: Send your digital files. ROE accepts IOS files from all major brands. To send directly from your scanner, search for ROE Dental Lab in your IOS portal. The exact protocol varies by manufacturer; ROE can walk you through it.

Step 4: If sending physical models, download and print a copy of your Rx to include with the case. A copy of your Rx is emailed to you automatically on submission.

Step 5: Ship at no charge. Visit www.roedentallab.com/UPS, click the UPS icon, complete your information, and click Process Shipment.

Step 6 (optional): Request supplies. ROE provides free boxes and supplies on request through the supplies request form. Supplies ship within 48 to 72 hours.
