What is the standard surgical workflow for an edentulous case?
For a fully edentulous case, ROE recommends the workflow below. Always follow your guided kit and implant company's protocol alongside these steps.
| Step | Instruction |
|---|---|
| 1 | Inspect the guide for proper implant site placement, as well as proper design and sizing for any instrumentation. |
| 2 | Flap the tissue adequately for guide seating directly on the bone. |
| 3 | Seat the guide onto the bone surface. Minor bone adjustments may be necessary to fully seat the guide. If fixation pins are included, firmly hold the guide in position, drill the buccal holes, and place the pins through the guide. If the guide has integrated teeth (as the denture scan appliance), close the patient into the bite to set the fixation. Without fixation screws or pins, firmly hold the guide in position. |
| 4 | Drill the osteotomies to completion following the provided drill report and the guided kit and implant company's protocol. If a fully guided kit is utilized and allows implant placement through the guide, proceed. If a fully guided kit is not utilized, remove the guide and place the implants. |
| 5 | Remove the guide. |
| 6 | If bone reduction is planned, this guide may be used to drill osteotomies and place implants prior to or after bone reduction, depending on the prolongation capabilities of the guided kit and how the guide was requested. |
A critical checkpoint is Step 3: achieving full, stable seating on the bone. Because visual references are limited after flap reflection, confirming the seat and using fixation are essential to accuracy.
