What is the CombiGuide and what problem does it solve?

The GuidedSMILE CombiGuide™ is a full-arch surgical guide system from ROE Dental Laboratory. It is built to solve the two issues most often missed in freehand full-arch surgery: adequate planned bone reduction and precise implant placement for predictable prosthetic outcomes.

Freehand full-arch surgery is popular, but it frequently fails to remove a planned, verified amount of bone. Without enough vertical space, the final prosthesis is compromised over the long term. The CombiGuide removes that guesswork by providing physical, stackable guidance for both bone reduction and implant positioning.

It is a stackable system made up of five components that work together, and it is fixated to the bone after the tissue is reflected. The components are described below.

Component Role in the workflow
1. Pin Guide The delivery device. It is tooth-borne or tissue-borne with windows for the teeth, and it accurately positions and delivers the fixation base before being removed.
2. Fixation Base The anchor that is pinned to the bone and remains in the mouth the entire procedure. The other guides clip into it. It also serves as the bone reduction reference.
3. Bone Reduction Guide Establishes the level to which bone is reduced, delivering the planned amount of vertical space.
4. Osteotomy Guide Controls drilling and implant placement, including rotation indexing for correct implant angulation.
5. Carrier Guide Confirms bone reduction is adequate. When it seats passively, the surgeon knows enough bone has been removed.
