What do I do if I forgot my username or password?

If you have forgotten your portal username or password, call our team and provide your office phone number or the doctor's name so we can locate your account. Once we find your account, we can reset your credentials and send a password reset email to the email address on file.

If you are unsure of your username, we can look it up and confirm it over the phone. For example, we might tell you, "Your username is McCaffrey21."

We can also reset your password to be the same as your username for convenience, which you can then change to a preferred password once you successfully log in.
