My Pin Guide won't fully seat — how do I diagnose and fix it?
Full seating is critical; it sets the foundation for the entire surgery. Use fit indicator and confirm contact through the occlusal windows. Because of tooth undercut, not every window has to seat — just the occlusal/incisal contact. Verify on the model first, then repeat intraorally. Work through the likely causes below before adjusting aggressively.
| Cause | What to do |
|---|---|
| Guide not fully seated in windows | Use indicator and adjust until the occlusal/incisal windows contact the teeth. Confirm on the model, then intraorally. |
| Mobile teeth | Mobile teeth may need to be manipulated into the guide the same way they were impressed. If most or all teeth are mobile, flag this early in planning — ROE can build a Pin Guide with the opposing bite integrated so the patient closes into it while the facial pins are placed. (Lowers: use the tissue; uppers: use the palate for stability.) |
| Dental work since the initial impressions | Modify the guide, or extract teeth that don't affect seating. |
| Teeth that need removal | Check the case notes/GSI form — extractions for malocclusion or draw are noted there. |
| Vestibular tissue blocking seating | Flap the tissue (flap earlier on this arch) until the guide seats. Usually means the original impression didn't capture the full vestibule, or bone reduction extends past the vestibule. Inspect and adjust after seating. |
| Still won't seat after adjusting | The model may be inaccurate or the guide may be fabricated with errors. The case must start with a fully seated Pin Guide — it may need to be delayed. This is a clinical call based on how far off it is. |
When is "too much adjustment" too much? If aggressive adjusting clearly changes the fit and seating accuracy of the Pin Guide, that alone can be reason to stop and capture new records to start over.
