Important notices:
  • Please do not schedule an online meeting until you have received an email notification from us.
  • In the Name field below enter your name and that of the patient in this format: ‘Doctor Name’ / ‘Patient Name’

 

Once you complete this form:

  • The meeting is scheduled in the CHROME staff member’s calendar
  • An email confirmation is sent to your email address
  • A meeting invitation is emailed to you that contains a MS Teams web conference link.
  • At the time of the meeting click the link in the meeting information and join the meeting.
